Who is responsible for your information
Dr Steve Conteh is the data controller for enquiries submitted through drsteveconteh.co.uk. Contact: assiststeve7@gmail.com.
Where you go on to become a patient, your clinical records are held by the dental practice at which you are treated, and that practice acts as data controller for those records under its own privacy notice.
What is collected and why
The enquiry and referral forms collect your name, email address, telephone number, preferred location and whatever you choose to write in the message field. This information is used solely to respond to your enquiry and, if you proceed, to arrange an appointment. The lawful basis is your consent, and, once you become a patient, the provision of healthcare.
Please do not submit detailed health information through this website. If clinical detail is needed, it will be gathered securely at your appointment.
Who sees it
Enquiries are read by Dr Steve Conteh and by his practice assistant, who manages the diary. Where an appointment is arranged, relevant details are passed to the reception team at the practice concerned. Information is not sold, and is not shared with any third party for marketing.
How long it is kept
Enquiries that do not lead to an appointment are deleted within twelve months. Where you become a patient, your records are retained by the treating practice in accordance with UK dental record retention requirements.
Your rights
You have the right to ask for a copy of the information held about you, to have inaccurate information corrected, to withdraw consent, and to ask for your information to be erased where there is no legal reason to keep it. Requests can be made to assiststeve7@gmail.com.
If you are not satisfied with how a request has been handled, you may complain to the Information Commissioner's Office at ico.org.uk.
Cookies
This website sets no tracking or advertising cookies. Fonts are loaded from Google Fonts, which receives your IP address as part of that request. If analytics are added in future, this notice will be updated and consent obtained before any tracking cookie is set.
Security
The site is served over an encrypted connection. Form submissions are transmitted securely and stored in access-controlled systems. No system is completely secure, which is why the forms on this site are deliberately designed not to collect clinical information.
